Terms and Conditions

THIS PAGE STATES THE TERMS AND CONDITIONS UNDER WHICH YOU MAY HAVE ACCESS TO VIRTUAL DENTAL SCREENINGS THROUGH quip. PLEASE READ THIS AGREEMENT CAREFULLY BEFORE COMPLETING A VIRTUAL DENTAL SCREENING.

1. Virtual Dental Screenings received through the quip platform are a covered benefit available to primary enrollees and their dependents enrolled in Delta Dental PPO™ or Delta Dental Premier® plans administered by Delta Dental of California and affiliates ("Delta Dental").
2. A Virtual Dental Screening received through quip is applied as a covered diagnostic assessment under your existing Delta Dental plan and is not subject to your plan’s frequency limitations. Consults will apply toward cost sharing and annual maximums. If the Virtual Dental Screening is not eligible for coverage because you are not an enrollee or a dependent in a Delta Dental PPO™ or Delta Dental Premier® plan, or if you have exceeded your annual maximum, you have the option to access the service by paying the required assessment fee directly through the quip platform.
3. If the quip Independent Dentist suggests further treatments or referrals as a result of the Virtual Dental Screening, and you choose to receive further treatment or schedule a follow up in-person dental visit, those services will be subject to your Delta Dental plan benefits coverage.
4. All quip Independent Dentists who provide Virtual Dental Screenings to Delta Dental enrollees or dependents are part of Delta Dental’s PPO provider network.
5. A quip Independent Dentist is solely responsible for his or her provision of services, including Virtual Dental Screenings, and compliance with the requirements applicable to his or her professional license (including compliance with applicable laws and standards of practice).
6. Directories of Delta Dental PPO network providers will be available after the Virtual Dental Screening assessment report is delivered to the enrollee or dependent but can also be found online at deltadentalins.com
7. Coinsurance costs provided by quip are an estimate only and may not include the application of your eligible dental plan benefits. Actual costs may vary based upon your specific Delta Dental plan benefits.
8. Delta Dental may revise these terms and conditions at any time by updating this posting.

Delta Dental will not be liable for any damages of any kind arising from the use of the quip platform, including, but not limited to, direct, indirect, special, incidental, exemplary, punitive, and consequential damages, whether in an action under contract, negligence or any other theory, arising out of or in connection with the use, inability to use, or performance of the quip platform, or any information, services, products and materials available on or via the quip platform.
